Lead Case Worker

Location: Atlantic City, NJ  |  Position Status: Full Time  |  Position Shift: Days
Reports To: Director, Hospital-Based Violence Intervention Program

Trainings: Annual, CPI, CPR

Position Summary
The hospital-based violence intervention lead is a 12-month position responsible for the direct coordination and administration of services provided by AtlantiCare's hospital-based Violence Intervention Program, which includes bedside visits and intensive follow-up case management services to individuals who have been hospitalized for violent injuries. This position requires some nights, weekends and swing shift and requires on-call and in-person crisis response.

Duties and Responsibilities
  • Supports the organization's mission, vision and values.
  • Flexibility to work evenings and weekends.
  • Ability to work with poise and professionalism in stressful situations.
  • Works closely with senior therapist and program director to coordinate case management services for participants in crisis.
  • Effectively develops, collaborates and maintains excellent relationships with Juvenile Justice, victim-serving institutions and other co-providers.
  • Conducts frequent (multiple times daily) reviews of hospital, probation and police data for potential referrals.
  • Assists in monitoring violence intervention specialists, which may include face-to-face contacts, case documentation, case reviews and weekly schedules.
  • Assists in training violence intervention specialists on program and organizational policy and procedures, completing intake paperwork and service plans, making required visits and providing adequate service and referrals to program participants.
  • Participates in weekly team meeting with intervention specialists to provide support and guidance regarding case plan implementation.
  • Assists intervention specialists in preparing for and participating in weekly case conferences.
  • Periodically monitors intervention specialists' implementation of service plans and case conference recommendations.
  • Facilitates field training of all newly hired intervention specialists.
  • Accompanies intervention specialists on field work as needed in order to provide assistance, give guidance/feedback and assess performance.
  • Accompanies intervention specialists when purchasing critical goods for program participants when purchases exceed the monetary limits allowed for the intervention specialists  and purchase is approved by program director as critical or necessary for participants.
  • Ensures intervention specialists submit appropriate receipts and documentation when providing monetary resources to participants.
  • Provides relevant updates and coordination across internal programs as needed regarding mutual clients and/or emerging situations.
  • Assists in monitoring efficiencies of intervention specialists, addressing concerns with supervisor when needed, and working closely with senior therapist and program director to address and resolve when possible.
  • Assists senior therapist and program director with identifying, interviewing and hiring intervention specialists appropriate for the positions.

Minimal Qualifications:
  • Bachelor's degree in human services and 2-3 years of experience with similar populations.
  • Knowledge of intensive case management.
  • Knowledge and comfort with geographic areas and cultural or linguistic match to community and program participants.
  • Strong documentation and/or written and oral skills.

Preferred:
  • Strong case management experience required (minimally 3 years).
  • Experience with case conferencing.
  • Strong documentation and/or written and oral skills.
  • Demonstrated commitment to working with youth and families.
  • Experience with trauma-informed care and violence intervention work is a plus.

Licensure:
Valid New Jersey driver's license.

Equipment Use: Computer, copier, facsimile and shredder. Driving record to pass check and ability to drive in day/night and reasonable weather conditions.
